Complete the form, including noting which type of supporting documentation you will include.

Save the form to your computer and then upload it to us online via secure messaging in the Message Center. You can reach the Message Center by clicking the envelope icon at the top of any page and choosing "Take me to Message Center."

Click "Upload Document" and choose the saved form, along with copies of your supporting documentation. Schwab will review your documentation and inform you when your account's name change has been completed.
